Private Pay
A caregiver will come into
your home to provide any of the following services:
-
Personal Care-
Assists member with activities of
daily living, maintaining personal cleanliness & meeting
essential personal physical needs. This may include;
Showering, Bathing, Toileting, Dressing, Grooming, Oral
Hygiene, Ambulation activities, Transferring to and from
wheelchair, Assisting with adaptive equipment.
Preparing & Serving food, encouraging to eat, Assisting
with self-administration of medications, etc.

Light Housekeeping-
This
service is only performed in clients areas of the home
or common areas of the home used by the client. It is
providing routine household activities at a client's
place of residence. This may include: Dusting, cleaning
floors, bathrooms, oven, refrigerator, and windows (if
necessary for safe and sanitary living conditions).
Cleaning kitchen, washing dishes, routine maintenance &
cleaning of household appliances, Changing linens &
making beds, Washing, drying & folding laundry, shopping
for and storing household supplies & medications, Taking
out garbage, etc.
*If the
client lives by themselves we may assist them in all
areas of their home.

Errands-This
is doing routine chores for the member. This may include
going to the post office, grocery store, picking up
prescriptions, or any other routine community activities
-
Respite-
Short
term care & supervision of the client. Supplementing
care and/or to support or relieve caregivers for the
benefit of the client. This may include: providing
relief to family members/caregiver, provide supervision
at all times for client, provide for social, emotional,
& physical needs, ensure medications are taken on time,
provide for first aid & appropriate attention to
injury/illness, assist with transportation in all daily
living activities i.e. employment site, day treatment,
medical appointments, visiting with family/friends.
HCBS
Services through DES/DDD
A caregiver will come and
provide contracted services in your home. These could include
some of the following:

Respite-
Short
term care & supervision of the client. Supplementing
care and/or to support or relieve caregivers for the
benefit of the client. This may include: providing
relief to family members/caregiver, provide supervision
at all times for client, provide for social, emotional,
& physical needs, ensure medications are taken on time,
provide for first aid & appropriate attention to
injury/illness, assist with transportation in all daily
living activities i.e. employment site, day treatment,
medical appointments, visiting with family/friends.

Habilitation-
Provides interventions to maximize the
functioning of our clients. This can include
habilitative therapies, special development skills,
sensory-motor development & behavior intervention. This
may include: Strategies to address behavioral concerns,
Implementing recommendations from speech, physical, &
occupational therapies, mobility training, alternative
or adaptive communication training, self help skills
(bathing, combing hair, brushing teeth, etc.) Self help
skills ( laundry, cooking, cleaning etc.) Exercise
programs, Money management, banking, shopping, Community
survival skills, stranger/danger skills. etc.

Attendant Care-
Is service that
provides routine homemaking, personal care, respite
care, general supervision, and companionship for
extended time periods to members for the purpose of
keeping members well integrated with their families.

Job
Follow Along-
Once a client has obtained
employment, Job Follow Along may entail, being at the
place of employment, helping the client with any issues
that may arise with management or co-workers. Following
through to make sure the client understands their
duties, and addressing any issues the client may have
with their new employment.
Adult
& Child Development Home
This is where a person or
couple opens up their home and can have up to 3 persons with
developmental disabilities move into their home. The home will
have licensing standards to follow with full support from People
First. Training will occur in the home for the individual to
help them become more independent.
Vocational Rehabilitation
Transportation- To and from
assessment sites, college classes, employment etc.
Adoption Subsidy
Respite- This can include
meals, hygiene, taking to appointments, arts & crafts, community
outings etc.
